    YOUR FIRST VISIT EXPERIENCE

      From the first day you join our office, we will cater to your dental needs.  On your first visit we will conduct a comprehensive examination and a periodontal evaluation. Full mouth x-rays are necessary in order to fully examine you, as it gives us a picture of the tooth structure above and below the gum line, as well as enables us to evaluate the bone structure surrounding your teeth.

      After gathering all of the necessary information, your treatment will be reviewed with you. This allows you and Dr. Stevens to develop a personalized plan for your oral care. We will work with you so that your oral care plan will be convenient for your lifestyle and busy schedule. We look forward to helping you maintain a happy, healthy smile.

      We are always accepting new patients. We accept referrals from our loyal patients as well as others who have heard about us through word of mouth. Please call us to schedule an evaluation appointment.

      I just wanted to let Dr. Stevens know how Lauren did yesterday after having her 2 teeth removed.  She cried on the way home, I think  more from the shock that they were gone, but once we got home she was so happy.  She watched tv for about an hour and then she went swimming and was playing like nothing happened.  She ate like crazy too.         So that we may all enjoy a smooth working relationship, we ask all new patients to take a few minutes to read our basic office policies and fill out the registration and history forms prior to our appointment. These PDF forms can be found below and downloaded to your computer, so you can easily print them. After filling out these forms, you may e-mail them to our office or bring them with you to your appointment. This will expedite your wait time and allow us to spend more time with you.

        Financial / Insurance Information

        Dental treatment is an excellent investment in an individual’s medical and psychological well being.  Financial considerations should not be an obstacle to obtaining this important health service.  We feel that our patients deserve the best dental care we can provide. We also feel that everyone benefits when definite financial arrangements are agreed upon at the start of treatment.

        Payment is expected at the time of service for any charges not covered by insurance, such as deductible amounts, co-pay amounts and non-covered services.  In order that we may have a definite understanding regarding the payment of dental fees, our office offers the following payment options:

        Cash – includes cash, personal checks or money orders. Credit/Debit Cards – our office accepts all major credit cards, including Visa, Mastercard, Discover, and American Express. We also accept any debit cards with the Visa or Mastercard logo. CareCredit – we offer this separate line of credit to cover your entire family’s health care needs. Because CareCredit can offer more flexible payment options than we can, it has replaced our open-billing policy. Upon approval, a credit line will be established. CareCredit offers interest-free payment plans up to 12 months and extended payment plans with 11.9% interest for up to 60 months. There is no annual fee or membership fee. Financing with CareCredit gives you the ability to begin your treatment immediately and spread the cost over a period of time.

          Dental Insurance Coverage

          If you are incorporated with a traditional insurance plan, typically one in which you can choose any dentist you desire, we will be more than happy to assist you by filing your insurance claim. Our practice is not involved with any PPO or DMO insurance plans other than Delta Premier.  If you aren't sure of the type of plan you possess, we will gladly call for you and explain your benefits to the best of our ability.

          In order to provide you with premium dental care, and to illuminate our position on filing for your insurance benefits, we would like to share some facts concerning dental insurance with you:

          • The basis of your dental insurance plan is a contract between your employer and the insurance company. While we endeavor to estimate your dental benefits to the best of our ability, this is an estimate ONLY. This estimate should not be relied on as the ultimate decision. It is advised to contact your insurance company directly if questions should arise.
          • You could potentially receive notification from your insurance company stating dental fees are “higher than usual and customary” (UCR),  because insurance companies survey a geographic area, calculates and average fee and considers it customary. Since the survey includes discount clinics and managed care facilities, the average is less. Plenty of private practice doctors will have “above usual and customary” fees.
          • Several plans indicate participants will be covered “up from 80% to 100%”, but do not clearly specify plan fee schedule allowances, annual maximums or limitations. Insurance companies will NOT cover all dental services.

          We ask that payment of your deductible and estimated co-payment be paid at the time treatment is rendered.  Please understand that this is only an estimate and is based on the information available to us. If, after 90 days, your insurance company has not paid their portion, the patient will be responsible for paying the balance in full, and any balance remaining on your account at that time.
